Wauseon Location

Construction was completed in the Fall of 2016 on our practice location at 1090 North Shoop Avenue in Wauseon, Ohio. This office replaced our previous office which was located at the Fulton County Health Center. Dr. Kouba, Dr. Dollman and two physician assistants are currently seeing patients there and New Patients are welcome
